The intended parents must send the birth declaration to the civil status registrar, accompanied by an authentic copy of the notarized surrogacy contract and a copy of the surrogate mother's consent or, depending on the situation, a document stating the death or a certificate concluding that the latter is unfit.

Upon receipt of the documents, the civil status registrar draws up the birth certificate with the names of the intended parents, if everything is in order. The intended parents thus become the legal parents, without the need for the adoption process.
