XI. Treatment of COVID-19 patients with convalescent plasma

Since Behring and Kitasato reported the therapeutic effects of diphtheria antitoxin plasma in 1891, plasma therapy has become an important tool for immunotherapy of acute infectious disease pathogens. This new infectious disease progresses rapidly in severe and critically ill patients. In the early stage, pathogens directly damage target organs and then cause severe immunopathological damage. Passive immune antibodies can effectively and directly neutralize pathogens, which reduces target organ damage and then blocks subsequent immunopathological damage. During multiple global pandemic outbreaks, WHO also emphasized that “convalescent plasma is one of the most recommended potential treatments and has been used in other epidemic outbreaks.” After the COVID-19 outbreak, the initial mortality was quite high due to the lack of specific and effective treatments. Since the mortality rate is an important indicator that the public is concerned about, in order to avoid public panic, clinical treatment that can effectively reduce the mortality rate in critical cases is key. As a provincial hospital in Zhejiang Province, we are responsible for the treatment of patients from Hangzhou and critically ill patients in this province. In our hospital, there are many potential convalescent plasma donors and critically ill patients who need plasma treatment.

1 Plasma collection

1.1 Donors

It should be noted that when comparing the virus neutralization titer and the quantitative luminescent determination of IgG antibodies, we found that the presented detection of specific IgG antibodies against SARS-CoV-2 does not fully demonstrate the actual ability of plasma to neutralize the virus. Therefore, we proposed the virus neutralization test, or the total antibody level at a 320-fold dilution of plasma, as the first option.
